At Granada Hills Dental Group, we want your first visit to be smooth and welcoming. To save time at your appointment, our team can email your new patient paperwork ahead of time, or you're welcome to arrive a few minutes early and complete everything comfortably in our office.

Please bring a photo ID and your dental insurance card (if applicable) to your first visit. If you'd like your forms in advance, just let us know when you schedule and we'll send them over.
