THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Granada Hills Dental Group is committed to protecting the privacy of your protected health information (“PHI”). We are required by law to maintain the privacy of your PHI, to provide you with this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect.

How We May Use and Disclose Your Health Information

Treatment. We use your health information to provide and coordinate the dental care and treatment you receive, including sharing information with other providers involved in your care.

Payment. We may use and disclose your health information to bill and obtain payment for the services we provide, such as submitting claims to your dental insurance.

Healthcare Operations. We may use your health information for activities necessary to run our practice, such as quality assessment, staff training, and appointment reminders.

Other Permitted and Required Uses and Disclosures

We may use or disclose your health information without your authorization in limited circumstances permitted or required by law — for example, for public health activities, to report abuse or neglect, for health oversight activities, in response to a court order or lawful process, to avert a serious threat to health or safety, or as otherwise required by law. Other uses and disclosures will be made only with your written authorization, which you may revoke at any time.

Your Rights Regarding Your Health Information

You have the right to:

  • Request to inspect and obtain a copy of your health records;
  • Request a correction or amendment to your health information;
  • Request restrictions on certain uses and disclosures of your information;
  • Request to receive confidential communications by alternative means;
  • Receive an accounting of certain disclosures of your information;
  • Receive a paper copy of this Notice upon request; and
  • File a complaint if you believe your privacy rights have been violated.

Changes to This Notice

We reserve the right to change this Notice and to make the revised Notice effective for health information we already have as well as any information we receive in the future. The current Notice will be posted in our office and on our website.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our office or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
